You probably already know of the Skylab “mutiny” or “controversy.”
On Skylab 4, a complete set of rookie astronauts — Jerry Carr, Ed Gibson, and Bill Pogue — were given a very aggressive schedule that they just could not keep up with. Expectations were high after the likes of Pete Conrad and Alan Bean, and these guys were all adjusting to being in space for the first time and ground control was pushing them.

They basically quit talking to mission control for a day. They spent that day adjusting to their new work environment and not doing much work. The following day they continued with their 84 day mission, and came to an understanding with ground control.
You may also be aware that Apollo 7 was a difficult situation between mission control and Wally Schirra. He refused to do tests and television programs that had not been planned, rehearsed or trained for. Of course, Wally had a bad cold and was irritable. When he refused to wear his helmet during re-entry, they were beyond upset. Wally later made a television commercial for Actifed with the tag line (while while pointing to an Apollo helmet) “Have you ever tried to sneeze in one of these?” He won a Clio Award for that.
NASA made sure that none of the above crew members ever flew again, even crew members that were not directly involved (Walt Cunningham and Don Eisele).

I think honorable mention might go to STS-27. At launch, foam caused a lot of damage to the thermal protection system of the orbiter Atlantis. Cmdr. Robert “Hoot” Gibson asked for and received permission to use a camera on the robotic arm to examine the damage. What he saw alarmed him. He concluded that they would never survive re-entry. Because of the classified mission, NASA would not allow unencrypted video to mission control. The encrypted video was poor resolution and it did not look alarming to mission control and they dismissed the concerns in their assessment. Gibson was enraged and was prepared to tell NASA what they could do with their assessment on an open mike at the first sign of trouble with re-entry. Luckily, they survived.
